Continuing Education Verification

All dental professionals licensed in the state of Vermont are responsible for uploading the verification of their Continuing Education credits in any given licensure cycle. Below you will find the applicable CE Verification forms for the current license renewal cycle from October 1, 2021 - September 30, 2023.

Please print any that apply to you for courses that you attended. Be sure to complete the forms in their entirety including the unique verification code provided for each program you attended.
